J-1 Visa Sponsorship Application CAMPGROUP, LLC - International Staffing Office CampGroup, LLC uses the information on this application to process the DS 2019 forms. You will need the sponsorship verification forms (DS 2019) to apply for your J-1 work Visa. Information must be entered prior to printing the application. Please take the time to carefully enter information to prevent miscommunication and errors. DS 2019 forms with incorrect information may be denied by the embassy processing your visa or at the United States port of entry. The DS 2019 forms will be delivered to you by courier. A physical address is needed to send the visa. A Post Office box address will not be accepted. This application is for people that have secured employment at one of our summer camps.
